Most of those are fictional, or bands, or in the case of Dr Martens, he was a real doctor.
However, Dr John is another to add to my list - he was called Dr John for decades and given an honorary doctorate in 2013.
There are 8 at my local GP practice. Each one is a Bachelor of Medicine and a Bachelor of Surgery but none has a doctorate.
They are honorary titles for medics without an MD or PhD
What about people with PhD's who call themselves Doctor like Dr Goebbels who wrote a doctorate on romantic poetry (snigger); strange cases such as Dr Goldsmith who spent years as medical student; Dr Johnson who was given a doctorate for his dictionary;Dr Crippen an American homeopath who could not and did not practise medicine in UK. I fear none of these fulfil your very exacting criteria.
its easy you just change your name by deed poll to Dr then your real name.
mind you it does confuse the postman when you get mail addressed to Mr Dr Jim

Goebbels doesn't count. It's a PhD, so you can call yourself (officially) Dr, even though people think you're a nob if you.
